B. Scheduling and Patient Intake Information (Acuity Scheduling)
We use Acuity Scheduling (“Acuity”) to let you request and book appointments and, where applicable, complete scheduling-related questions/forms.

Because email is not always a secure way to transmit sensitive information, you should not include sensitive health details in the website contact form.
7) ACUITY SCHEDULING (THIRD-PARTY SCHEDULING)
We use Acuity Scheduling to manage appointment requests and bookings and, where applicable, to collect scheduling/intake details.
Information you submit through Acuity is processed by Acuity and is subject to Acuity’s and its provider’s policies and settings. We encourage you to review their privacy practices:

8) HEALTH INFORMATION AND HIPAA
